GLP-1 Medication Tax Deduction Rules and Eligibility for 2026

A significant portion of the adult population in the United States uses GLP-1 medications, according to a recent KFF Health Tracking poll. The cost for these treatments can be substantial, often reaching over a thousand dollars monthly for those without insurance coverage, and several hundred dollars even with it.

While these drugs may be claimed as a medical expense deduction on federal taxes, several specific conditions must be met for the claim to be valid. Taxpayers must choose to itemize their deductions, and this strategy is only financially beneficial if the total of itemized deductions exceeds the standard deduction amount set for their filing status.

Furthermore, only medical expenses that exceed 7.5% of an individual's adjusted gross income are eligible for deduction. The expenses must also be for costs that were not reimbursed by insurance or another program, and they must be classified by the Internal Revenue Service as qualified medical expenses.

Qualifying for the Deduction

For a GLP-1 drug to be considered a qualified medical expense, a licensed medical provider must have diagnosed the patient with a specific disease. The medication must then be prescribed explicitly to treat that diagnosed condition. Examples of qualifying diagnoses include type 2 diabetes and obesity, which the IRS recognizes as a chronic medical condition.

Prescriptions issued solely for general weight loss, without an underlying disease diagnosis, do not qualify for the tax deduction. This distinction often applies to medications obtained through certain telehealth services where a formal diagnosis may not be established.

Using Health Accounts and Insurance Considerations

Funds from a Health Savings Account or a Flexible Spending Account can be used to pay for these medications, but only under the same condition: the drug must be prescribed to treat a diagnosed disease. Expenses paid with these accounts cannot also be claimed as a tax deduction. Patients may need to provide a letter of medical necessity from their provider to use these accounts for reimbursement.

When insurance covers part of the medication cost, only the out-of-pocket portion paid by the patient, such as a co-pay, is potentially deductible, subject to the other applicable rules.

Overall Tax Impact

For most taxpayers, taking the standard deduction provides greater tax savings than itemizing deductions, including medical expenses. Contributing to an HSA, if eligible, often provides a more direct tax benefit, as contributions reduce taxable income and withdrawals for qualified medical expenses are not taxed.
